Waiver and Release of Liability

I, the undersigned participant (or parent/legal guardian of a minor participant), hereby acknowledge and agree to the following terms in consideration of being permitted to participate in martial arts training and related activities at Reisky Martial Arts, located at Unit #210, 12960 84th Avenue, Surrey, BC.

Acknowledgment of Risks

I understand and acknowledge that martial arts training, including but not limited to karate, arnis, sparring, weapons training, and conditioning exercises, involves inherent risks that may result in serious personal injury or property damage. These risks include, but are not limited to:

• Bruises, cuts, and abrasions
• Strains, sprains, and muscle injuries
• Joint injuries (fingers, wrists, ankles, knees)
• Impacts from strikes, kicks, or training equipment
• Injuries from falls or being thrown
• Injuries from contact with training partners or equipment
• Head injuries including concussions
• In rare cases, permanent disability or death

I acknowledge that this list is not exhaustive and that other injuries, both foreseeable and unforeseeable, may occur.

Assumption of Risk

I voluntarily assume all risks associated with participation in martial arts training at Reisky Martial Arts. I understand that:

• Martial arts is a physically demanding and potentially dangerous activity
• Training involves physical contact with other participants
• Despite the use of protective equipment and safety protocols, injuries may still occur
• I am responsible for my own safety and must train within my physical capabilities
• I must immediately inform instructors of any injuries or physical limitations

Medical Fitness

I represent and warrant that:

• I am physically fit and have no medical conditions that would prevent safe participation in martial arts training
• I have consulted with a physician if I have any doubts about my ability to participate safely
• I will immediately disclose to instructors any medical conditions, injuries, or medications that may affect my training
• I will not participate if I am ill, injured, or impaired by alcohol or drugs
• I am responsible for my own health insurance and medical expenses

Release and Waiver of Liability

In consideration of being permitted to participate in training, I hereby release, waive, discharge, and covenant not to sue Reisky Martial Arts, its owners, instructors, employees, volunteers, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, damage, or death that may be sustained by me or any property belonging to me, whether caused by:

• The negligence of the Released Parties
• The actions or inactions of other participants
• The condition or use of the facility or equipment
• Any other cause whatsoever

This release includes injuries or damages caused by the ordinary negligence of the Released Parties.

Indemnification

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs (including attorney fees) they may incur due to my participation in training, whether caused by my own actions or the actions of others.

Photographic Release

I grant Reisky Martial Arts permission to use photographs or video recordings of me (or my minor child) taken during training or events for promotional purposes, including but not limited to the website, social media, and marketing materials, unless I specifically opt out by notifying the school in writing.

Emergency Medical Treatment

I authorize Reisky Martial Arts and its representatives to obtain emergency medical treatment for me (or my minor child) if necessary. I understand that I am financially responsible for any medical expenses incurred.

Agreement to Follow Rules

I agree to:

• Follow all safety rules and instructions provided by instructors
• Use required protective equipment when specified
• Train in a controlled and respectful manner
• Not use techniques learned in training for purposes other than legitimate self-defense
• Comply with all dojo rules and etiquette

Severability

If any portion of this agreement is held invalid or unenforceable, the remaining portions shall continue in full legal force and effect.

Governing Law

This agreement shall be governed by and construed in accordance with the laws of the Province of British Columbia and the laws of Canada applicable therein.

Acknowledgment of Understanding

I have read this waiver carefully and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Reisky Martial Arts. I sign it of my own free will without any inducement.

How to Sign

This waiver must be signed in person at the dojo before your first class. Please arrive 15 minutes early to complete registration paperwork, including this waiver.
